Why does this happen?

I used to see that from time to time, but I thought Ken made a change to address it and have not seen it since. (Powerful placebo effect perhaps?)

Here’s a workaround when it does happen: menu View -> Toolbar. toggle it off then back on.

I can’t reproduce this, but I added a couple small things to try and
prevent it.
